Study name: REDUCING TUMOR BURDEN IN PATIENTS WITH STAGE IV EGFR POSITIVE NON-SMALL CELL LUNG CANCER: SAFETY AND FEASIBILITY TRIAL (TITANS TRIAL)

Histology NSCLC, only non-squamous
Tumor stage Stage III - IV
Host / recruiting site 1 Antoni van Leeuwenhoek Enrollment Recruiting
Therapy line First line (1L)
Design

Single-center, open-label, non-randomized phase I/II safety and feasibility trial.

Intervention

Tumor burden reduction combining standard targeted therapy (EGFR-TKI, osimertinib) with early local consolidative therapy (such as stereotactic radiotherapy/SABR or surgery) to remaining tumor sites.

Key outcome parameters

Feasibility and safety (measured by toxicity scores and successful completion of the local treatment).
Progression-free survival (PFS), local tumor control, and overall survival

Key inclusion criteria

• Confirmed diagnosis of Stage IV non-squamous NSCLC.
• Documented activating EGFR mutation (e.g., exon 19 deletion or L858R).
• WHO/ECOG 0–1

Key exclusion criteria

• Prior systemic therapy for stage IV
• Severe, uncontrolled comorbidities (e.g., severe interstitial lung disease, unstable cardiac conditions).
• Pure squamous cell histology
